(with travel dates, mandatory )

Personal Last 3 month bank statement , (copy)
Return Ticket,
Salary slips last 3 months, (copy)
Original Foreign Exchange Receipt OR credit card copy with last statement,
original Travel Insurance for Minimum 15 days with insurance company stamp,
Hotel Confirmation. (mandatory )
In case the visa is requested for continuous stay which exceeds 30 days, invitation from Italian company should be completed with a declaration that:

If minor is traveling WITHOUT OR with single parents then NOC in affidavit format on Rs. 100 stamp paper stating traveling with parents/relatives/friends for tourism which should be notarized and attested by home ministry
